Westlake Chiropractic is a paperless office. You will not have to sit and fill out paperwork prior to your visit. Instead, we ask that you click on the Link below and begin filling out your data immediately. When you arrive at the office, all of your data will already be in our system and we can  get you started on your personal care solution. If you are an auto or work comp patient, there is extra paperwork to print out as indicated below.
